Obtain A Repeat Prescription?

Prescriptions can also be ordered via the form below.
Please allow 48 hours to arrange; don't leave your request for a prescription until it is too late. This especially applies at public holiday times when five days' notification is required. Requests can be posted to the surgery, handed in to the receptionists or telephoned (if you are housebound or have transport difficulties) to the prescription desk between 9.00am - 12 noon and 1.30 - 3.00pm Monday, Tuesday, Thursday and 9.00am - 12.30pm and 2.00 - 3.00pm on Wednesday. Prescription requests cannot be taken through our main switchboard, so if this dedicated phone line is engaged, please phone later. It is very important to use the prescription slip to identify the medication and indicate the chemist to be used. The prescription will be sent to the chemist of your choice.
Prescription Delivering
If you are housebound or have transport difficulties some local chemists
will deliver your prescriptions. Please state if you wish to use this service.
Please check your medication when you receive it, as there can be no redress
once accepted.
